Award Information
There will be five categories of awards that will be presented at the 57th Annual LASN Convention.
Newsletter Articles (no application necessary) Newsletter articles will be judged by the Horizon’s Editor
Membership Awards (no application necessary) Membership Awards will be judged by the BTN/ Membership Chair.
Scrapbook Awards (no application necessary) Scrapbook Awards will be judged by exhibitors.
Image of Nursing Awards (required application here) Image of Nursing Awards will be judged by the Awards Committee and the Image of Nursing Chair.

Community Health Award
Purpose: To reward LASN chapter organizations for their outstanding contributions to the community. This application is used for the greatest number of community service projects and for best overall community service project.
Criteria:
1. This application must be typed or generated on your computer from the forms provided through the LASN website (www.lasn.org).
2. Using LASN provided format please document each event separately.
3. If event lasts over several days use only one form and enter dates under project date(s).
4. If the event is a continuing project that occurs more than once over the semester or year please use only one form and state this in the date portion of the application. Also provide the number of total occasions that this project was performed under project date(s).
5. Deadline for completed applications must be postmarked by September 9, 2011. Make two copies of the completed application. keep one for yourself and send the other copy to the following address:

Results will not be discussed prior to the 2011 Convention. All submitted materials will be kept confidential by the Awards Committee unless written permission specifically states otherwise.
